Pathways from Waitlists to Person-Centered Supports for People with Intellectual and Developmental Disabilities (PATH-IDD)
Many people with intellectual and developmental disabilities (I/DD) wait months or even years to receive the home and community based services (HCBS) they need to live healthy, independent lives. These long waitlists delay access to person-centered supports, creating significant barriers to good health, community living, and full participation.
PATH-IDD Project Partners
The PATH-IDD project is working to change that. We are building evidence to identify effective and fair healthcare policies that strengthen Medicaid HCBS programs and improve outcomes for people with I/DD.
PATH-IDD Objectives
Working closely with our partners—including community members and state advisory boards—PATH-IDD is developing a sustainable, multi pronged approach to:
- Examine how Medicaid policies affect waitlists, services, health, and community life for people with I/DD;
- Develop and Test a person-centered tool that helps people with I/DD and their families share their needs and helps states better manage waitlists and improve services; and
- Create easy-to-use resources to help people with I/DD and their families understand and navigate HCBS.
Through this work, PATH-IDD will generate actionable insights and tools to improve Medicaid HCBS and promote person-centered planning, and community integration for people with I/DD.
